Investigation Summary
Employee #1, a part-time maintenance man for Sea Products Co., which operates a seafood processing plant, and a coworker were installing piping for an overhead sprinkler system. Employee #1 asked the coworker to raise him to the ceiling using a Nissan forklift and a pallet box. When the coworker tried to lower Employee #1, the fork chains stuck and, because the lowering handle was still activated, slack built up in the chain. Employee #1 shook the pallet box and the chain came free. The forks, pallet box, and Employee #1 fell to the floor. He sustained a dislocated ankle with multiple fractures that took eleven screws and a plate to repair. The cause of the accident was a malfunction in the forklift mast operation and an untrained person operating the forklift. The operator was not authorized to operate the forklift and the employer did not know he was operating it. The employer was cited for five general violations, including the unauthorized operator.
