Investigation Summary

Investigation Nr: 170723845
Event: 10/16/1993
Employee's fingers amputated when caught in equipment

At 11:00 a.m. on October 16, 1993, Employee #1 had the tips of his right index and middle fingers amputated when his fingers became caught in the nip point of a v-belt and pulley drive of a small walnut sweep machine. He had been attempting to take the slack out of the v-belt drives while the machine was running. When he got the belt too tight, the belts began to turn without the slave pulley engaged and his fingers were pulled into the nip point. The v-belt drives were said to be guarded, but a small opening on the back side could have allowed access to the nip point. After the accident, Employee #1 was taken to the employer's residence, where a family member (and EMT) took him to Ride Out Emergency Hospital in Marysville. He was treated and released. Employee #1 is a high school student who performs this work on weekends.
