Employee injured in fall from roof

Employee #1 was walking on a beam 24 ft above the ground without a safety belt and lanyard or a place to tie off. He was handing material to a coworker on the roof of a three-story house. Employee #1 stepped either from the beam to a scaffold that belonged to another trade, or from the scaffold to a beam. He said that a board slid on the scaffold, causing him to fall inside of the beam 11 ft to the balcony. He landed on his tailbone, which came through the skin. Employee #1 lost spinal fluid and required surgery. The employer installing a metal roof did not properly judge the safety hazards; Employee #1 should have been tied off.
