Investigation Summary
At about 9:30 a.m. on July 6, 1992, Employee #1, of Hastie's Capitol Sand & Gravel, Folsom, CA, was cleaning a 1 cubic yard concrete mixer. The employee started cleaning the hopper of the concrete batch plant. He climbed inside the hopper, which has a single shaft with multiple paddles for mixing the concrete. Using an air chisel, he proceeded to chip the concrete build-up from the sides. When he got to the back side of the electrical control, the impact and vibration started the mixer (the controls were not locked out). One paddle came around, catching Employee #1's left arm at the elbow and removing all the flesh down to the wrist. As the paddles continued to go around, his left shoe was torn in half and the foot was cut. The machine was stopped when it caught him in the upper body and right shoulder, pinning him against the wall of the mixer. Fellow employees heard his screams for help and came to his aid. 911 was called and paramedics arrived within minutes. Employee #1 was transported by Life Flight to UC Davis Medical Center, where he was treated for a degloved left arm, dislocated right shoulder, and other multiple cuts and abrasions.
