Investigation Summary

Investigation Nr: 170716393
Event: 08/01/1992
Employee suffers compound fractures to spine from fall

At 11:16 a.m. on August 1, 1992, Employee #1 was injured when the Hitch Hiker 3600R lift truck he was operating suffered a mechanical failure. The hydraulic ram, which raises and lowers the fork truck to the load carrying truck bed, became unthreaded from the pin that secures the hydraulic ram to the mast. The weight of the fork truck exerted leverage sufficient to disengage the ram from the mast, allowing the vehicle to free fall to the ground. Employee #1 was belted to the operator's seat and suffered compression fractures to his spine from the impact of the fall. He was hospitalized.
