Employee's hand crushed in mast of forklift

At approximately 2:30 p.m. on November 30, 1992, Employee #1, of the County of Nevada, Department of Transportation, was a member of a maintenance crew that was attempting to place a large chip box on the back of a flat bed truck using a forklift. When the crew was unable to lift the box from the end, the forklift operator moved from the rear of the box around toward the side. During the move, Employee #1 was riding next to the operator. When the forklift approached the side, the operator started checking the fork position by looking along the left side. Employee #1, on the right side, stood up and approached the mast, in preparation to help attach the chains. While standing behind the mast, he placed his right hand on the main mast cross brace. The operator, when approaching that side, lowered the forks to place them under the side of the box. The forks were lowered and a cross piece on the inter moving section came down onto Employee #1's right hand, severely crushing and breaking the hand. He was immediately driven to Sierra Nevada Hospital, then transferred to Roseville General Hospital for surgery.
