Employee's ear severed by conveyor equipment

At about 3:12 p.m. on May 20, 1994, Employee #1 was part of a cleanup crew that was removing metal debris from an area around a conveyor belt that was carrying scrap metal. A coworker who had been working in the same area heard a cry for help and saw Employee #1 waving his arms. The coworker went to the other side of the conveyor belt to investigate and found that the Employee #1 had been struck on the head by the equipment and that part of his left ear had been severed. He was hospitalized.
