EMPLOYEE FELL FROM THE EDGE OF A ROOF AND IS KILLED

At about 7:30 a.m. on May 17, 1994, Employee #1, an employee for Peninsula Custom Homes, Incorporated, and a coworker climbed to the roof of a two-story wood-framed house under construction. They needed to cover some roof openings with tarpaulins because it was raining. In doing so, Employee #1 slipped on the wet roof surface and fell to the ground. He received head injuries that eventually led to his death. Neither man was wearing a safety belt.
