Employee's finger amputated by forklift's safety cable

Employee #1 was disconnecting a safety cable that extended from a forklift to a conveyor while the forklift engine was still running. The cable jerked back and his finger was caught and crushed between the tow bar eyelet and the axle frame of the conveyor system. Employee #1's right middle finger to the 1st knuckle was amputated.
