Employee's fingers amputated in belt and pulley

Employee #1 is employed as a machine operator on a lineal grooving machine. This machine utilizes a belt and pulley to maintain pressure on materials during its folding operation. The belt drive is a flat belt with numerous v configurations that ride in a v-belt pulley. On Employee #1's machine, the back of the pulley housing was not guarded. The belt was worn and was continually coming off. Employee #1 had put it back on several times. As the belt once again began to work its way off, Employee #1 tried to push it back on with his hand while the machine was running. His fingers became caught between the belt and the end of the pulley. It severed the first and middle fingers of his left hand to the first knuckle.
