Investigation Summary
A tree trimmer, certified through the International Society of Arborists, had ascended a ladder to access an area of a tree that needed to be trimmed. The employee climbed into the crotch of the tree at a height of about 9 meters above the ground and started to tie himself in with his lanyard. He slung the lanyard around the trunk; but, when he attempted to catch the end, he lost his balance and fell to the ground. He sustained a broken arm, back, and neck. He was hospitalized for his injuries.
