Investigation Summary
Employee #1 was engaged in the construction of a hillside vineyard with three other employees. They were riding in the bucket of a Caterpillar 930 articulated front loader without seats or seat belts. The loader went out of control on a 30 percent slope with a loose shale surface in the new vineyard. Employee #1 was ejected from the bucket, run over by the loader, and killed.
