Investigation Summary

At about 3:15 p.m. on May 20, 1993, Employee #1 was attending a meeting in a trailer with about 20 other employees. When Employee #1 opened the trailer door, a cloud of gas descended on the trailer. He began coughing and experiencing breathing difficulties. Employee #1 then collapsed to the ground. Fellow employees carried him to a second room and administered air from a SCBA in the trailer. He was also given oxygen from a tank brought to the area by other employees. Although his coughing decreased, Employee #1 began having chest pains and pains in his right arm. He was transported to the hospital by a helicopter. He was examined and released the same day.
