Employee struck in head by flying plexiglass piece

On April 14, 1993, an accident occurred injuring one employee at the Masonite Corporation plant located in Ukiah, California. Employee #1 was at the end of his shift recording the readings on the presses when he observed that press #2's pressure was not coming down from 2,900 psi. He opened two pilot valves to reduce the pressure. As the pressure fell to approximately 1,000 psi, the 26 inches by 8 inches by 5/8 inch plexiglass sight window in the holding tank blew out. A piece struck him on the left side of his head.
