Investigation Summary
Employee #1 was working on a scaffold 36 in. wide, 84 in. high, and approximately 20 ft long (2 by 10 ft sections) while painting the side of a new building at a construction site. He was descending the scaffold when the scaffold fell over. The scaffold was not required to be secured at one end. Employee #1 fell off the scaffold onto a parapet wall and the scaffold fell on top of him. He broke his right arm and was released the same day from the hospital.
