Employee injured when boom collapses

Employee #1 was on a platform attached to a vertical oil rig boom and was tied off. A section of the boom retracted and the platform dropped approximately 30 feet without warning. Employee #1 suffered a head injury, a fractured foot, and a punctured lung caused by the impact against the platform. The boom extension lock had failed.
