Investigation Summary
At approximately 7:30 a.m. on April 23, 1996, Employee #1, the maintenance supervisor, was performing his usual duties of greasing, oiling, and lubricating the various component parts of his company's conveyor belt assembly systems. The system was fully operating when Employee #1 saw some trash on one of the up-take roller assemblies. He placed his right hand into the nip-point range of the unguarded roller assembly, which was approximately 33 in. above floor level. His hand and arm were immediately pulled into the take-up roller of the horizontal conveyor belt system. Employee #1 sustained a crushed right hand and upper arm, as well as extensive lacerations.
