Employee's fingers amputated when caught in pie maker

Employee #1 and coworkers were using a Colborne pie machine to prepare pie crusts. She was pushing the dough toward the rollers that would flatten it so that it could be placed onto the pie tins. The guard to the rollers was in place, and the opening between the table and guard was 3/4 in. The dough was 3/4 in. high and was not sliding into the rollers as Employee #1 wished, so she used her right hand to push it under the guard opening and into the operating rollers. Her right hand entered the 1/8 in. roller opening, crushing two of her fingers which were then amputated when they were pulled out from between the rollers. Employee #1 had previously been told to use her left hand and to keep it away from the opening.
