Investigation Summary
At approximately 9:00 a.m. on February 4, 1995, Employee #1, a 27-year-old baker with more than 6 years experience, had been filling trays with muffin batter from a Hinds-Bock muffin depositor, model #SP8W, serial #2149. He was attempting to unclog batter from the dispenser when he struck the enclosed switch, activating the equipment. The tip of Employee #1's finger was partially amputated. The employer had provided a scraper to remove the batter and the equipment had warning signs posted on it. Employee #1 failed to follow the employer's written policy to unplug the equipment and bleed the air line.
