Investigation Summary
At about 1:00 p.m. on February 16, 1995, Employee #1, the senior drill operator of Ofiaro Building & Engineering, Berkeley, CA, with more than 15 years work experience, was using a crowbar to clear rocks from a pier hole. He was sitting on the edge of the hole. The core-barrel head suddenly dropped, pinning Employee #1 between the machine and the ground. He sustained a fractured right wrist, and bruises and contusions to his back and face. Employee #1 had not secured the portable hydraulic drilling head with a 0.375-in. chain and a 4 by 6 beam before working under it, as he had been instructed. He had drilled six or more pier holes at the accident site and many more at other job sites.
