Employee struck in face as table saw kicks back

At approximately 9:00 a.m. on April 7, 1994, Employee #1, a maintenance worker with 4 months of employment experience at a health club, was cutting blocks of wood on an unguarded 10 inch table saw, model T8294, serial #0004, 4.1 hp motor. He was not wearing eye/face protection. He states that he was instructed to use a push stick. Employee #1 failed to push the stock by hand at the end of the cutting stroke, nor did he change the saw's position so the stock could be pulled. This caused the stock to kick back and strike him in the face. Employee #1 suffered contusions on his face and corneal abrasions with vitreous hematoma to his right eye. Accident-related violations are T8CCR 3382(a), serious, and T8CCR 4302(a), serious.
