Investigation Summary
Employee #1 was attempting to clear a "bridge" on the feed end of a plastic extrusion press with his bare hands when recycled plastic bags, the feed stock, wrapped around his right hand and pulled it through the hopper and into the pressure screw's point of operation. Employee #1 could not extract his hand or shut off the press, and his coworker did not know how to turn the press off. The owner and two other employees came in response to Employee #1's cries and turned off the press. They could not extract his hand until they manually turned the drive pulley in reverse. Four of Employee #1's fingers were amputated, and the thumb was later surgically removed. The surrounding tissue on the employee's hand was burned. Employee #1 asked a coworker to wrap his hand in a plastic bag. The owner suggested that Employee #1 raise his arm while applying pressure. Employee #1 walked outside to wait for an ambulance, which he said arrived 25 minutes later. He was transported to the Highland Hospital emergency room.
