Investigation Summary
The Employee #1, a handyman, got into the crate of a forklift and then asked a coworker to get on the forklift and lift him up. While the crate was elevated approximately 10 to 12 feet, Employee #1 began to move around in the crate which caused it to tip over. He fell to the concrete surface and died of injuries to his head and neck.
