Investigation Summary
On August 25, 2003, a painter was "caulking" the underside of a sloped roof. He was standing at the roof's edge, working approximately 10 ft, 8 in. above a tile and concrete surface. He lost his balance and fell to the ground. A coworker found him lying unconscious in a pool of blood. Emergency medical service was called and he was pronounced dead at the scene. The employee received blunt force head trauma. He was not wearing fall protection equipment, and his foreman did not enforce the use of it.
