Employee killed in trench cave-in

On June 1, 2000, Employee #1 was working with a crew that was excavating a joint utility trench for a new building under construction. He was using a shovel at the edge of the trench while a coworker was operating a backhoe. The coworker later stated that he was moving the backhoe when he saw some movement out of the corner of his eye. He then observed Employee #1 in the trench, pinned against the wall by an apparent cave-in. The coworker and other crew members attempted to rescue Employee #1, but he had been killed.
