Investigation Summary
At approximately 9:30 a.m. on April 9, 2000, Employee #1 and two coworkers started their work. They drove to the work site at approximately 9:40 a.m. and started removing bolts and screws from the roll stand machine. They placed a chain with two hooks around the machine in order to secure it on a forklift. Forks were raised under the machine; Employee #1 climbed on the forklift frame and stood on a mast to push the hanging overhead florescent light (approximately 7-8 ft above the concrete floor) to the side because it was making contact with forklift. Employee #1 used his left hand to push the light to the side while a coworker operated the control to lower the telescoping mast and move forklift back. Employee #1's right foot was crushed while standing between the mast uprights. He was hospitalized.
