Empolyee Injured In Fall From Ladder

Employee #1 received trauma to the head from apparent fall. A coworker was nailing cross bracing near a hatch opening when he heard a noise and found Employee #1 laying on his left side on the concrete floor. Employee #1 had his right foot lodged into the fourth rung from the bottom of a fixed wood ladder. Employee #1 also had a 4-ft by 4-ft by 0.75-in. plywood board lying on top of him. Employee #1 was bleeding from his head and unconscious. The coworker stated that the Employee #1 was to bring plywood from a stack and placed it against wall, and then hand up to the coworker. Employee #1 was hospitalized with severe brain damage. There were no witnesses to the accident.
