Investigation Summary

Investigation Nr: 202311916
Event: 08/18/1999
Employee severs finger with a cutter

At approximately 11:00 a.m., Employee #1 was operating a cutter to excise ties from a stack of combined tie patterns when he placed his hand in direct contact with the blade and amputated his finger. During the excising of the ties, he made one or two passes on the material. Being right handed, he was cutting from the right side. He then cut the machine off and positioned it to his left, and proceeded to turn the patterns around to a new cutting angle. In doing so, he brought his right index finger into contact with the blade of the tie cutter while it was still rotating. He was transported to the hospital where he underwent surgery to repair the damage to his finger.
