Employee Suffers Amputation

Employee #1 was washing down the sprocket and chain drive assembly of a food processor machine using a hose. The chain drive was in full operational mode and the cover guard had been removed in accordance with a company authorized protocol. The existing company cleaning protocol required that Employee #1 was to use gloves and was to extend his hand into the "pinch point" area to hose down food particles that had become stuck to the machinery during its normal operating period. Employee #1's right hand glove became caught in the moving chain drive. Employee #1 called for help several times but no one heard him because of the noise of nearby machinery in operation. Finally, a coworker saw that Employee #1 was in difficulty and came to his immediate aid. Employee #1 was stuck in the machine for approximately 45 minutes. Employee #1 suffered the amputation of part of his right index finger. Employee #1 was hospitalized for his injuries.
