Investigation Summary
At approximately 3:30 p.m., Employee #1 was unloading produce from his truck. He was working very swiftly and was walking backward pulling a dolly loaded with produce towards the loading dock. He failed to realize that he was at the lift gate edge and stepped backwards off of the truck. He fell approximately 4.5 ft to concrete grade and was hospitalized with a fracture to his head.
