Investigation Summary

Investigation Nr: 202312559
Event: 06/07/2000
Employee 's Leg Is Injured by Falling Metal Plate

At approximately 9:30 a.m., Employee #1 was standing in the vicinity of a free-unsecured relatively large piece of metal (ramp) plating which was being welded upon by Employee #1. The piece being worked on was extremely unwieldy and poorly balanced on end. It was approximately 1/4 in. thick and weighed an estimated 400 lbs. and was not blocked or secured in any manner to prevent it from falling over. When Employee #2 went to make a cut on the vertical side nearest him; a piece of 2 by 2 tubing fell away from the plate - causing the larger metal plating to shift weight and topple over onto Employee #1. He was not hospitalized.
