Investigation Summary
Employee #1 was working on a flat roof completing the roof sheeting. Employee #1 and his partner were cutting and filling in the irregular shaped holes left to be covered. Employee #1 had been wearing a harness, lanyard, and lifeline attached to an anchor line. Employee #1 disconnected the lanyard from the safety line in order to walk to the center of the roof and a greater distance than his fall protection would allow. Employee #1 fell 30 ft through a 48 in. by 30 in. by 20 in. wide roof opening when carrying a plywood sheet back to his work site. Employee #1 did not stop to reconnecting to the safety line. Employee #1's view of his walking surface was blocked by the plywood he was carrying. Employee #1 was hospitalized for his injuries.
