EMPLOYEE FELL FROM LADDER

On December 9, 1997, Employee #1 was assigned to clean up the parking lot behind buildings Number 1 and Number 2. When he finished the clean up, he continued his prior assignment of repairing and replacing overhead lights. Employee #1 needed a particular part that he thought was on the mezzanine. To gain access to the mezzanine, Employee #1 unlocked the door to the janitors' closet, turned on the light and climbed a fixed wooden ladder. The mezzanine was approximately 10 ft off the floor. Employee #1 did not recall how he fell, or from where the fall was initiated, whether from the ladder or the mezzanine. Employee #1 did not remember striking the light fixture on the way down. However, it was not operational when he was found on the floor of the janitor closet. Employee #1 sustained fracture of the head and was hospitalized.
