Investigation Summary
On April 14, 1999, Employee #1 was operating an order picker truck in a distribution warehouse of a home furnishings retailer. Employee #1 fell 20 ft, 7 in. from the elevated operator platform to the warehouse floor. His tether/lanyard was not attached to the platform.
