EMPLOYEE'S FINGER TIP IS AMPUTATED IN BALL VALVE

On February 25, 1998, Employee #1, a line operator, working for Oatey Company, was working in the Cement Department. He was trying to close the faucet on a Simplex dispensing machine and he inadvertently stuck his finger into a 0.25 in. opening, cutting the tip off. He was transported to Eden Hospital and hospitalized. The Division was not notified of the accident until May 7, 1998 when the investigation was initiated.
