Employee injured when struck by mold cart and blank molds

At approximately 3:25 p.m. on September 15, 1995, Employee #1 was pulling a mold cart loaded with approximately 15 blank molds that had just been taken out of the #13 preheat oven and loaded on the cart for mold change on the hot end glass forming machine changer. The cart's wheel became caught in a hole in the floor and it tipped over onto Employee #1, who had fallen as the cart tipped. The blank molds, weighing approximately 30 lb each, also fell onto him. Employee #1 was transported to St. Rose Hospital for treatment of multiple fractures to his ankles and right little finger.
