Event: 02/19/2008
Employee Is Killed in Fall in Garage Under Construction

At approximately 7:20 a.m. on February 19, 2008, Employee #1 was removing wood formwork from the side of a wall on the third floor of a parking garage. He fell approximately 36 ft to the basement level and was killed. Employee #1 had fall protection equipment on but was not tied off. It appeared that a 4 in. by 4 in. joist used for false work had failed in the area directly below the wood panel that Employee #1 was removing; a portion of this joist landed on the ground alongside him. There were no witnesses to the accident; however, several other employees heard a noise they described as a ladder striking the ground. Upon investigating, they found Employee #1 unconscious and breathing. Emergency responders pronounced him dead at the scene. According to the coroner, Employee #1 struck his head when he hit the floor, causing head trauma and a fractured back. The cause of the fall was not determined.
