Investigation Summary
On June 15, 2005, Employee #1, a warehouse worker, was helping to load carpet padding onto a truck. A coworker was driving a Clark forklift while Employee #1 was walking on the right side of the forklift beside upright metal racks. The forks were approximately 5 inches above the concrete floor. Employee #1 stepped onto the right fork of the forklift to ride to where the pads were stored. His foot slipped off the fork, he fell onto the concrete surface, and his left leg was run over by the right front tire of the forklift. Employee #1's left leg was fractured, and his great toe was amputated. Employee #1 was hospitalized.
