Investigation Summary

Investigation Nr: 202315552
Event: 08/12/2005
Employee Is Killed When Electrocuted by Power Source

On August 12, 2005, Employee #1, of ESR Manor, was alone watering his plants at the back of the adult residential facility. He apparently came in contact with a power source and was killed when he was electrocuted. Upon inspection, it was noted the exposed ground along the fence and the walkway had been saturated from water for long period of time. A check inside the tool storage box, where electrical tools were stored, had no signs of electrical arching. The electrical panel located at the north side of the house was also inspected and had no signs of arching; however, the grounding was insufficient.
