Investigation Summary

At approximately 9:15 a.m. on June 6, 2005, Employee #1, a plasterer hose puller, was part of plastering gun crew. They were plastering a two story single family house working on a 3-tier metal scaffold. The scaffold uprights were plank with three 2-in. by 10 ft wood planks, and standard guardrail were installed. Employee #1 was on the second level, and a coworker was on the third level. The coworker handed Employee #1 the hose and nozzle. Employee #1 turned and either tripped or slipped on the planks and fell down under the guard railing and onto the ground below. Employee #1 was hospitalized with a fractured left arm.
