Employee injured after falling from on top of truck

At approximately 2:30 p.m. on January 15, 2003, Employee #1 was inspecting a previously installed rain gutter drainage system. He was standing on top of the storage bin of his utility truck and having some difficulty in being able to actually see what the problem was. To get a better view, Employee #1 turned on his heel, which caused him to lose his balance, slip and fall. He fell 4 ft 10 in. onto concrete grade. Employee #1 was hospitalized.
