Event: 12/10/2001
Employee injured from fall off of roof

At approximately 8:00 a.m. on December 10, 2001, Employee #1 and other coworkers were cleaning the gutters of an apartment building when Employee #1 slipped on an icy patch located on the roof. The icy patch resulted from several days of incremental cold weather. Employee #1 stepped upon the icy surface as he traversed a small parapet on the roof of the building. As he slipped, he lost his balance and slid down the 2 to 12 slope of the roof. He slid approximately 12 ft to the edge of the roof and fell approximately 15 ft to the ground.
