Employee injured when thumb amputated by vehicle fan

Employee #1 sustained a partial amputation of the right thumb, the entire top half of the thumb down to the first knuckle, when the loose end of a piece of banding cord which he had wrapped around his thumb fell through a small portal for motor oil entry, got caught in the vehicle's motor fan and Employee #1's thumb was pulled into the rotating mechanism. Employee #1 was hospitalized.
