Employee Fell from Second Floor Construction

On July 23, 1999, Employee #1 was performing his assigned duties on a construction site. Employee #1 accessed the second floor of the residence under construction by climbing a ladder. Employee #1 was attempting to climb down the ladder when the ladder slipped out of position. Employee #1 attempted to return to the second floor but was unable to fully return to the floor and fell 12 feet to the concrete floor. The investigation revealed that the ladder had been secured by two nails bent over the ladder rails and the ladder did not exceed three feet above the landing.
