Investigation Summary
At approximately 12:00 p.m. on August 14, 1999, Employee #1 had finished stacking pallets and was walking to the office area. At the same time, a coworker, who was operating a forklift, struck a stack of pallets causing the pallets to fall onto Employee #1's legs. The coworker was backing his forklift down an alley that was more than four ft wider than the forklift when he struck the 14 ft high stack of pallets with the back of the forklift. Employee #1 received a broken leg and was hospitalized.
