Investigation Summary
On August 18, 1999, Employee #1 was operating a Hyster Forklift. While driving the forklift, he proceeded around a corner at an excessive speed causing the vehicle to overturn. He fell out of the vehicle and its back tire ran over his left foot crushing it. Emergency medical personnel were summoned and took him to the hospital. He was hospitalized for two weeks and underwent orthopedic surgery to repair the bone, ligament, tendons, and muscle damage in his foot.
