Investigation Summary
On August 17, 2006, Employee #1, a teacher, was painting the wall area above the sink in her classroom. She mounted the sink, covered everything with newspaper and started painting. While on the counter, she answered a cell phone call, stepped into the covered sink, lost her balance and fell backward onto the floor. Employee #1 sustained a laceration to the back of her head, requiring several staples, but was not hospitalized. The employee had not been instructed to paint the wall.
