Employee's Finger Tip Is Amputated While Using Table Saw

On May 5, 2004, Employee #1 was working at a guarded Cantek 10 horsepower electric table saw, (Serial Number 100466, Model Number MBS 400). He was trained and checked out on proper use of the equipment and was unsupervised. He was pushing a 4 ft long by 8 ft wide by 3 ft thick piece of lumber through the guarded blade with a push stick held in his right hand. When the drop piece did not move on the left side of the blade, he reached in with his bare left hand to remove the remnant and contacted the blade with his left index finger. Employee #1 claims to have been distracted by his own concentration on his right hand and did not recognize the hazard to his unprotected left hand. His left index finger at the first joint was amputated.
