Employee Amputates Several Fingers in Conveyor

At approximately 1:00 a.m. on October 20, 2004, Employee #1 was standing on the left side of the out feed conveyor from the BOC freezer tunnel, Model KFT 36-10 CU, Serial Number D-30-12882, taking frozen dessert trays off the conveyor. There is a gap, 2.5-in., between the belt and the conveyor frame on each side. This gap is not guarded and allows access into the in-running pinch points on both sides at the locations where workers stand to remove the desserts. Employee #1 reached for a tray and her fingers went under the belt which took her right hand into the tail pulley. She attempted to pull her gloved right hand out of the machine with her left hand and got it caught between the conveyor belt and tail pulley. She yelled for a coworker to turn off the machine and then managed to pull both hands out. She suffered fractured fingers on both hands and was hospitalized.
