Investigation Summary

Investigation Nr: 170842884
Event: 09/20/2004
Employee'S Arm Is Injured When Caught In Machine

At approximately 7:45 a.m. on September 20, 2004, Employee #1 was helping to clear jammed material from a cyclone's vacuum ducting on the sorting line for recyclables at a refuse collection company. He was positioned on the recycling/sorting belt at the top, next to a 20 HP, 36-in.-diameter fan. Two coworkers had just cleared duct at a lower duct location and climbed back up the stairs to Employee #1's location. While Employee #1 was reaching through a 7-in. by 10-in. opening, a coworker activated the start button, starting the fan, which caught Employee #1's right hand and arm. He was hospitalized for a dislocated elbow, fractured hand, and a fractured lower arm.
